O registo contém nome, naturalidade, idade, estado civil, profissão, residência), juntamente com a data e hora do falecimento, a causa da morte, origem e duração da doença, o nome do cemitério para o sepultamento e as assinaturas do médico/cirurgião e do comissário de saúde da paróquia.
